Mutual Liability Waiver and Acknowledgment of Risk

This agreement releases SAS Training & Consulting LLC and/or P3 Training Group, LLC, as well as the hosting organization (collectively, the “Training Providers”), from any liability related to physical or emotional injuries that may occur during participation in the <COURSE NAME> training and live-action role-play scenarios (“Activities”) conducted at <LOCATION> on <TRAINING DATE>.
 

Acknowledgment of Nature of Activities

I understand that this training includes scenario-based role play that may involve simulated high-stress situations, including raised voices, sudden loud noises, and emotionally intense interactions. While no physical contact or weapons are used, these scenarios are designed to realistically prepare participants for crisis response and de-escalation, which can be triggering for some individuals.
 

I acknowledge that:

• I am participating voluntarily.

• I am in good health and not aware of any personal condition (physical, emotional, or psychological) that would prevent me from safely engaging in this type of training.

• I understand I may feel emotional discomfort or distress as part of the training, and I agree to take personal responsibility for my wellbeing during participation.

• I may step away or pause my participation if I become overwhelmed and will notify the instructor as needed.
   

Release of Liability

By signing this agreement, I agree to hold harmless and release the Training Providers from all liability—now and in the future—including financial responsibility for any injuries or effects I may experience, whether physical or emotional, and regardless of cause, including potential negligence.
 

I also understand that the instructor will serve as the designated safety officer for this training and will provide instructions and guidance to promote a safe environment. I agree to follow all safety-related instructions and ask for clarification when needed.
 

Prohibited Items

I certify that I will not carry or bring any weapons—including but not limited to firearms, tasers, knives, pepper spray, or similar items—into the training environment. If I inadvertently arrive with such items, I will secure them in my vehicle or notify the instructor for safe handling.
 

Signature and Delivery

This agreement may be signed either physically in person or digitally via our learning management system (LMS). If signed digitally:

• My action of checking the acknowledgment box, entering my full name, and the date constitutes my electronic signature and full acceptance of this agreement.

• A copy of the completed waiver will be available to me for download through the LMS for future reference.
